How personalised support and compassionate care transformed the life of a man battling dementia, loneliness, and health challenges

Situation

Ernesto, a South American man with dementia, hearing impairment, alcohol addiction and an eating disorder, spends most of his time in his flat. Formerly a Creative Director in the advertising industry, Ernesto enjoys his own company. He has two daughters in South America who call him weekly during his care visits or when his private cleaner is with him, as Ernesto was not confident in using his phone.

Challenge

Ernesto’s health issues, including dementia and hearing impairment, coupled with his alcohol addiction and eating disorder, made it difficult for him to manage daily life independently. Despite having a friend as an informal carer, Ernesto’s growing sense of loneliness and his need for consistent and respectful care required a comprehensive support plan.

Support

Hopscotch Homecare took time to understand Ernesto’s desires for dignity, respect and control over his care. The team conducted a thorough assessment of his needs, focusing on building a good relationship with his Care Workers. Ernesto was introduced to three Care Workers with whom he shared interests, such as football.

Initially, Ernesto’s care package included basic support, but it was gradually increased to help him go out more frequently. When he felt lonely despite these outings, a referral to a day centre was made, but Ernesto preferred not to attend after two visits. Instead, he expressed a desire to attend church, and a regular Care Worker now accompanies him on Sundays.

Following the death of Ernesto’s friend and informal carer, his dementia progressed rapidly. Hopscotch provided emotional support and accompanied Ernesto to the funeral. 

The care package was further expanded to include more outings to places like parks, cafés, shops, galleries and museums, with protocols in place to ensure Ernesto’s safety when out alone.
